What Is A Consortium Agreement Form

You must complete Section I of the Consortium Agreement Form and forward it to the grant office of the host institution to complete Section II. You must complete a registration form at another institution (PTE) in order to take courses at the host institution. An ETP is required for each course you wish to transfer to UMGC. For each semester you wish to take at a different institution, a new PTE form must be completed. Undergraduate and graduate students should contact their academic advisor to begin the process. The model is written for a so-called open consortium, i.e. the consortium acts as such towards the customer, but the model can also be useful for negotiating a contract for a “silent” consortium. Based on the above procedures, it is important to allow enough time for all these steps. Ideally, the form arrives well before the first day of payment for the term. Call the host school in advance and ask them what their procedures are for dealing with consortium agreements where they are the host school. Remedial courses and vocational/technical courses are not transferred and very few exceptions are allowed. You cannot use the classes you supervise for the consortium agreement. You also cannot repeat the courses with the consortium agreement without first contacting your advisor and the grants office. You must enroll at least halfway through ASU for the semester in question (6 undergraduate credits; 5 graduate credits).

Students in the ASU Nursing CEP (Concurrent Enrollment Program) with Arizona Community Colleges, the University of Mary, and the Mayo Clinic must enroll in at least one ASU class.
